We provide primary care for adults, children and infants at our Hayes Avenue location, and primary care for adults at our E. Water Street location, both in Sandusky Ohio.
You can print and fill out our New Patient Paperwork and submit to our office, or complete the forms when you arrive for your appointment.
All self-pay patients follow these additional steps prior to your first visit with us:
